Your Support Network
Sometimes we all need a little help! If you are new to the service dog world, just thinking about getting or training one, these forms can help you have a ready list of helpers and how to sit down with each one and discuss if/how they can help.
The first page is for your list of catagories or groups of people you know.
The second page is your list of people that fall into each catagory, so you would have a seperate page for each catagory.
The third page is for each seperate person on your lists. It has a suggested interview format so that you can sit down and talk with each person about if and how they can help you with your service dog.
The fourth page is your list of professionals in your life that you may want to discuss service dog etiquette and how they/their staff should interact with your service dog to make life easier for all concerned.
The fifth page is for your list of support services people, so that they will be better able to care for your service dog in a manner that won't be detrimental to his/her training.
With the 4th and 5th pages, you would use a page 3 for each one.
This can all go into your Service Dog Care packet, something that would be very handy to have! A little file folder from the dollar store can keep all your important service dog information in one location.
